The diagnosis and treatment of lower extremity radicular pain are crucial for ensuring optimal patient outcomes. One of the most common medical problems that cause decreased work competence and a heavy cost is radicular low back pain. In this article, we will explore the role of electrodiagnosis in diagnosing this condition.

History taking and physical examination are essential steps in diagnosing lower extremity radicular pain. Accurate diagnosis has a paramount important role in proper treatment planning. However, many patients with MRI-negative but symptomatic conditions may require additional diagnostic testing.

Electrodiagnosis, which includes nerve conduction velocity and electromyography tests, has an important diagnostic value in these cases. In fact, coordination rates of MRI and electrodiagnosis with clinical findings were 58.6% and 89.5%, respectively, indicating the high accuracy of electrodiagnostic testing.

A recent study involving 165 sciatalgic subjects aimed to evaluate the diagnostic accuracy of MRI and electrodiagnosis in lower extremity radicular pain in relation to history and clinical findings. The results showed that 104 cases (68.4%) had shown some type of abnormalities in both MRI and electrodiagnosis, while 30 (19.7%) had shown abnormality only in MRI, and 21 (13.8%) only in electrodiagnosis.

In terms of the distribution of radicular pain, the study found that 67 cases (44.1%) had radicular pain in the left lower limb, 46 (30.3%) in the right, and 39 (25.6%) in both lower limbs.

The comprehensive history and physical examination, combined with MRI scanning and electrodiagnostic tests, are essential for diagnosing lower extremity radicular pain accurately. The results of this study highlight the importance of electrodiagnosis in diagnosing patients with radicular low back pain, particularly those with MRI-negative but symptomatic conditions.
